Alternatively, you can print the form and complete by hand.

  • Once the form is completed, you need to print the form and sign it. Ensure when printing that your printer settings allow for the entire form to be printed. We cannot accept any kind of digital or Adobe generated signature. Then mail the form to the Ulster County BOE office.

  • To register you will need:
    • your New York State DMV issued driver license, permit or Non-Driver ID
      • it must be your most recently issued document – you will need the ID Number and document number (See Sample Documents)
    • the ZIP Code currently on record with the DMV 
    • the last 4 digits of your Social Security Number (SSN)

Eligibility

To register to vote in New York State you must:

  • be a U.S. citizen;
  • be 18 years old by December 31 of the year in which you file this form (note: you must be 18 years old by the date of the general, primary or other election in which you want to vote);
  • live at your present address at least 30 days before an election;
  • not be in prison for a felony conviction and;
  • not claim the right to vote elsewhere.
  • *NEW* pre-registration can begin at the age of 16
  • *NEW* voting rights restored to parolees, post-incarceration to facilitate community reintegration and participation in the civic process, rather than requiring a parolee to wait until he or she has been discharged from parole or reached the maximum expiration date of the underlying sentence. (legislation adopted to law, Chapter 103 Laws of NY 2021)

2023 Voter Registration, Enrollment, and Address Change

You must keep your voter registration address up to date. To update your voting address you must complete a new voter registration form.

The Political Party Change of Enrollment Deadline for 2023 Primary Elections is February 14, 2023 for the Board to receive an enrollment change. Any political Party change made after February 14, 2023 will not take effect until after the Primary Election of 2023.

June 17Voter registration deadline for Primary: Last day application must be received by board of elections to be eligible to vote in the primary election.

June 12 – Change of address for Primary received by this date must be processed.

October 28 – Voter registration deadline for General: Last day application must be received by board of elections to be eligible to vote in general election.

October 23 – Change of address for General received by this date must be processed.
